NOTICE OF TRANSFER OF PATIENT RECORDS
On April 1, 2022, the assets of N.K.S. Pharmacy Limited (“NKS Health”) were transferred to 193602 Canada Inc. (“Purchaser Pharmacy”), which is an affiliate of Green Shield Holdings Inc. and is a part of the Green Shield Canada group of companies. This transfer was part of the planned transition of NKS Health’s specialty pharmacy operations to the Purchaser Pharmacy.
As part of the above transfer, NKS Health transferred all its patient records to the Purchaser Pharmacy. NKS Health undertook various measures to ensure that the health records were transferred in compliance with its obligations under the Personal Health Information Protection Act, 2004 (“PHIPA”). As the new custodian of your health records, the Purchaser Pharmacy is, like NKS Health, required to collect, use, disclose, retain and dispose of health records in compliance with PHIPA.
If you are a current or former patient of NKS Health, your health records will become the responsibility of the Purchaser Pharmacy. Going forward, should you have any questions or requests regarding your health records, such as requests to access or correct your personal health information, please contact the Purchaser Pharmacy’s privacy officer:
